Creating a Form with a popup confirmation

Hi I have created a form with the popup confirmation dialog box added as per the help page.
the dialog box does not have any content

I have a success and error page and I have removed the check for "show dialog automatically"
I have checked the "use built in php form processer script"

My form page is php and I have uploaded it to my site but when I fill in the form and send nothing happens although I do get an email.
I do not get the dialog box informing me that the form has been sent.

sorry for the long wait in replying. All seems to be working now. I think I had an issue with the website refresh.
all good now.
