form does not send email

Hello guys.
Me here again, with a problem (probably a stupid question...): the form on the website below "does not send" emails to the customer. I tried with the host company (PHP was 5.6, I upgraded to 7.1), but they said that everything is working properly and that the problem must be "on the site". I did a test by sending email via Outlook and the client received it, so I imagine the problem is really in the form. Can you see something wrong from there?

I some times had this problem but can assure you that it may be the host server issue, wrong smtp information or setup or else it may be the form you made must be missing something important.
