Embed google forms

Quizz 13 March

I placed the embed code taken from the google form into a HTML.
On the site page I can see the form, I can check the checkboxes however after I hit submit Im getting a blank page.
It doesnt return the score and the feedback like it does when it is accessed directly from google forms.
Any clues what Im doing wrong ?

It still doesnt work

So yes you can see the form.
Hit random boxes till you get all the 10 answers.
Hit Submit

You will get a blank page.

But If you go directly to the google form and hit the boxes and hit submit you will get this page

Completing the quizz through my site and hitting submit wont get me to the above mentioned page so visitors cant see the score and the feedback

The behavior of the form has nothing to do with the software.
So, may need to contact Google about this.

The quiz worked for me but I scored terrible, 2 out of 10
