Create checklist

Just wondering if there is an option to create a checklist ?
I used to have a photographic memory but i ran out of film !!!

I don't really have an example but i would like to make an online checklist of to do things that a person can check off as they complete the item(s)
I used to have a photographic memory but i ran out of film !!!

I did go there and took a checklist and dumped it into the HTML container but when i make a selection it doesn't save that on my server.
Anyway that could be done ?

The url was just to show as a example which I can't help you with. But there is nothing to stop you using the already available form tools in the software to build a similar layout using a checkbox (for selection) and a textarea for the notes. If wishing to save to your server then you will need to implement a database for this.
