I've created a website using the candy template.

I also added a form functionality in the website, see below for the project

But somehow when I live tested it, it didnt work, return to a404 page (below)

Also, when I tick the "use built-in PHP form processor script, the related web page becomes fuzzy

The extension of the page must be set to 'php'.
