Update 20120930:
1.Forget to put in options for the Submit button. It's done now.
2.Bold font weight on title text.
3.Added left padding for the Value (red) text.
4. Added code so that dotted lines on submit button is no more.

Should be viwed in Firefox. This extension do not look god in IE8.
