A Simple Booking Form For A Hotel Website

Hi all,

Firstly I'm not sure if this post is in the right area as it's related to a form, so appologies if I'm in the wrong section.

Anyway, I have created a booking form that could be used in website creations involving booking a room. It's using a floating layer and I have made the form responsive to cover all viewing ports.
Looking for any feedback on this form creation good or bad.

Just some thoughts:

1. Your missing a 'departure date'. Based on this form, there is nothing showing the length of stay.

2. Most hotels already have check-in and check-out times established so I'm not sure asking for the 'arrival time' is relevant or not.

3. You have the number of guests but I know a lot of hotels break it down into number of Adults and Children (as sometimes children are free).

4. No option to book more than 1 room. # of rooms drop down?

5. You may want to consider adding a terms & conditions link before the submit button that they agree to by clicking the submit button.

You don't have the date field for either arrival or departure...
