Form Wizard

I try to send a mail with a "Button" "Form Wizard", but I receive an error message.

Not Found
The requested URL was not found on this server.
Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request.

How to send an email from the "Form Wizard"?

What are you exact settings?
What is the URL of the page?
What is the PHP code of the page?

I'm trying to build a "Form Wizerd" which contains:
1- trade name
2- contact name
3- phone number
4- email address
5- free text
6 - a send button (link to mail)
7 - a reset button
The page is PHP and there are no other forms on this page.
I defined "Validate" but it doesn't work.
For example if I define a maximum of 10 digits, I don't get an error message if there are more than 10 digits.
The "send button" does not work.

I try to define a form who is sending a mail to me withe the details of the form.

The form doesn't work because you have set it to use mailto which is incorrect please read the following:
