I could be wrong here... Not at all, Ron. As usual, you hit the nail on the head. My apologies, I found the 'problem'... me. I actually have two different master pages: one for the Index page which doesn't include a 'Home' button (which would be redundant), and a second for the other content pages....

I was afraid that might be the answer, since this would mean that I would need to add a page header menu object to each of the 20 webpages, which I was hoping to avoid. At least I now understand how the template is created, which I didn't before.

Most likely incorrect email settings on your web host server. You should be able to review the server's email logs to see if you are reaching it, and what if any, error messages are being triggered.

This is not (and has never been) the default behavior.
That is good to know, as it gives me something positive to go on. Let me do more on getting the settings worked out first. I don't want you to spend time unnecessarily if I did something obvious by mistake.
