Xin Yang the original author of Chained Selects has relaunched his website and updated the files:

This extension is a game changer when it comes to presenting endless links compacted into 2 or 3 drop downlist, magnificently simplified and brought to WWB by Addedum. I just wish it could be made more aesthetic and customizable in its button layouts.

The only difference in the new script is "enabled OPTGROUP support in Mac ... 2015-10-19" so not worthy of a re-work from my side.
