It looks fine now, but when I first viewed it wasn't set to .php. The Doctype doesn't matter in this instance, if you use a form or any script the uses PHP then the page extension must be set to .php to ensure thst it gets properly processed by your server.

This indicates that you have used the wrong page extension name. Please ensure that it is set to .php rather than .html or .htm Please also ensure that your host supports PHP as well.

Pablo, now I am not the same level of programming as yourself, I do understand Pascal and VB though. Could it be possible to use/add the likes of a tooltip to each item in a combobox?, so that on hover it will show a abbreviated/short description of the item. As mentioned I am not on the same level ...

Where can I download this extension again?
I am sorry but this extension by myself is no longer available. As you can see it was originally made for WB7 which is no longer supported. I can only suggest thay you follow the link by Pablo and remove the extension and replace with another.

it is this code that is invalid/incomplete due to it having no opening/closing tags! either fix or remove.

To add a screenshot you can upload to your host/server and provide a url or use one of the many free image hosts such as https://imgbb.com/
Note the screenshot I gave is of the index page were it can be clearly seen the code is incomplete hence why it is getting shown at the top of page.

Screenshots aren't really helpful!, it is always best to provide a url, or if that not possible then a basic copy of the project with the menu and the breakpoints used.
