If you make the email address user configurable then that will be a huge security risk.
Spammers/hackers can then abuse your form to send emails to any one.

If full width is used then buttons will use percentages for the width. The sum of all items may not be exactly 100% because of rounding errors in the browser.

This is possible with a combo box and the $MAILTO$ variable.

There are several mistakes in the page, caused by manually added HTML code: You have added <html lang="en"> between the head tag. This is invalid and should be removed. If you add an image then it must have some size, so the initial size of the image will be used across all breakpoints.
After placing the image you can change the size in each breakpoints.

I do not have an Android smartphone myself, but I have tested your website on my iPhone and it seems to work correct.
Maybe you have installed an ad (or popup) blocker in the browser?

Misschien heb je de Toolbox aangepast zodat deze extensies verborgen zijn?

You have two index pages on the server:

http://www.modestbravura.com/index.php

You will need to remove index.html

Thanks for sending the project.
The problem is related to your breakpoints.
The default view is smaller than the largest breakpoint (1200), so the browser will display this breakpoint.
In the 1200 breakpoints you have set the color of the layer to #87CEFA with opacity 65

Thanks for sharing the project.
However it seems to work correct for me. I can set a different size for the image in each breakpoint.

Do I need to do something special to make it not work?
